DoDDS-Korea Schools Registration Information and Forms
To register your newly arrived or newly eligible dependent children for school with the Korea DoDDS schools, please have the following documents available. If you are re-registering your child in the same complex your child attended last school year please see your child's school registrar.

Documents needed for registration:
- Current orders / Extension orders
- Immunization Records
- Letter of Employment/Copy of ID cards (civilians)
- Form 175-R/ Copy of ID Card (for contractors)
- Renewal Agreement Orders or Form 1190 / (LQA with dependents) signed by official

- Emergency contact information, including phone numbers and addresses (need 1 local contact & 1 permanent stateside contact)
- Previous School Records

Required for enrollment of dependents into DoDEA Schools. Provides record of student and sponsor demographic data used in the administration of school programs. Provides emergency contact, pertinent medical and other vital information. Please reference the DoDEA Form 600sup. |

Completion of this form is required for enrollment in DoD schools. The data collected is maintained for “Statistical Use Only” and
is protected in accordance with the Privacy Act (93579),
OMB Circular A108,
and DoD Directive 5400.11. |

The information will be used within the DoD to determine the services to be provided to a student to assist the child to receive a free appropriate public
education.

Students enrolling in DoDDS must meet specific immunization requirements. The Department of Defense Education Activity Immunization Requirements DoDEA Form 2942.0-M-F1, 07 June 06, lists the minimum requirement and do not necessarily reflect the optimal immunization status of a student. This form is used to monitor students' health for learning. The information is used by school personnel to provide appropriate educational and student health services. |

Completion of this form provides permission for us to include a student, staff member or community member in print publications,
television, multimedia
or the Internet, permission is needed. |

The information on this form is used to authorize an individual student to use
government-owned computer resources in accordance with, and subject to enforcement provisions of, DoD and
DoDEA policies governing computer and Internet usage. |

Our primary concern during emergencies/early closings is to have your child arrive home safely. Please complete this form for each of your children. |

Sure Start is an early
intervention program for families in our school community with priority given to children
whose sponsor’s rank is in the E1 to E4 or GS1 to GS4, (GS/National Security
Personnel System (**NSPS, pay band 1, rank equivalent); or *Non ppropriated Fund
(NAF), NAF1 or NAF2 range, living and working at military installations overseas. Any
sponsor whose rank is E5 to E9 or GS5 to GS9; or NSPS (pay band 1, rank
equivalent) or NAF3 - NAF4, rank equivalent, who wishes to apply for the program, is
eligible to do so – with the understanding that priority is given to students whose
sponsor is the first eligibility category. The Sure Start Advisory committee, comprised
of school and community members, reviews applications for enrollment. Priority in
selecting 18 children to enroll in the program is given to dependents in the first
category. Children must be four years of age by 1 September in the school year in
which they enroll. |
